电针及强制性运动疗法对偏瘫上肢功能的作用

摘    要:目的比较电针、强制性运动疗法、及电针结合强制性运动疗法对偏瘫上肢运动功能的治疗作用。方法对象为门诊及住院患者,分为A组——强制性运动疗法组(10名),B组——电针治疗组(10名),C组——电针结合强制性运动疗法组(9名)。脑卒中患者生命体征平稳后即开始康复训练6周。在治疗前及治疗6周后分别进行Wolf运动功能试验(WMFT)、Fugl-Meyer上肢部分评分(FMA)、改良Barthel指数(BI)评定。结果三组患者治疗6周后的WMFT、FMA、改良Barthel指数均较治疗前提高(P<0.05);强制性运动疗法的WMFT、FMA、改良Barthel指数高于电针治疗组,电针结合强制性运动疗法组的WMFT、FMA、改良Barthel指数均高于其他两组(P<0.05)。结论电针、强制性运动疗法均利于脑卒中早期上肢运动功能恢复;二者结合对脑卒中早期上肢运动功能的恢复有叠加效应,亦是具有中国特色的康复治疗方法。

Abstract:Objective To compare the effects of paralyzed upper limb movement function treated by electric acupuncture,constraint-induced movement therapy,or combining constraint-induced movement with electric acupuncture.Method Outpatients and inpatients were classified to group A-constraint-induced movement therapy(10 cases),group B-electric acupuncture(10 cases),group C-combing electric acupuncture with constraint-induced movement therapy(9 cases).Six-week rehabilitation was started just after the patients’ vital signs were stable.WMFT,FMA and modified BI evaluations were employed pre-treatment and after six-week treatment.Results In-group comparisons:WMFT,FMA and modified BI were all increased in three groups(P<0.05).Between-group comparisons:WMFT,FMA and modified BI of constraint-induced movement therapy were more different than electric acupuncture,while WMFT,FMA and modified BI in combing electric acupuncture with constraint-induced movement therapy were the most different than the others(P<0.05).Conclusion Electric acupuncture and constraint-induced movement therapy promote upper limb movement function,combing electric acupuncture with constraint-induced movement therapy in recovery of upper limb movement function exists superimposed effect in earlier stage of hemiparalysis patients,and it is of typical Chinese character.
