CT介入下突出物内外注射胶原酶治疗非包含型腰椎间盘突出症64例分析

目的：研究CT介入下突出物内外注射胶原酶治疗非包含型腰椎间盘突出症的有效性。方法：64例非包含型腰椎间盘突出症患者，在CT介入下行针内针技术经小关节内侧缘穿刺达突出物内外，分别于突出物内外注射胶原酶200—600u、600—1200u。观察患者术后6天、1月、6月疗效及相关并发症，术后6月以上复查CT行突出物缩小程度评定。结果：术后6天、1月、6月优良率分别为56．2％、85．9％、95．3％。突出物缩小程度：重度：73．1％；中度：19．2％；轻度：7．7％。结论：CT介入下突出物内外注射胶原酶治疗非包含型腰椎间盘突出症疗效好，尤其适合突出较大患者，术后影像改变明显，并发症少。

The clinical analysis of CT guiding combined injection of collagenase in treating uncontained lumbar disc herniation in 64 patients

Objective：To explore the validity of CT guiding combined injection of collagenase in treating nncontained lumbar disc herniation. Methods：64 patients suffering from lumbral disc herniation were reported. Guided by CT puncture was performed through inner margin of facet joint. Collagenase 200- 600u,600 - 1200u were injected into and the surface of prominence respectively. The effect and complication of 6 days,1 month ,6 months were observed and the solution rates of prominence were assessed by CT scanning over 6 months after treatment. Result：The effective rate of treatment were 56.2%, 85.9%, 95.3%, respectively in 6 days, 1 month, 6 months after treatment, with the solution rates of prominence were 73.1%, 19.2% and 7.7% for heavy, moderate, mild. Conclusion：The clinical effect of CT guiding combined injection of collagenase in treating uncontained lumbar disc herniation was good. This method was especially fit for major prominence , imageology changes were clearly and complications were less.
