广州地区产IMP-9金属酶铜绿假单胞菌的再调查

摘    要:目的 调查广州地区部分医院2007年10月到2008年11月的铜绿假单胞菌泛耐株,筛榆出其中产IMP-9金属酶的菌株并分析其同源性,对比2005-2007年初广州地区多家医院曾出现的产IMP-9金属酶铜绿假单胞菌的克隆株,分析近期广州地区是否还有该流行株的存在.方法 Q-PCR筛选产IMP-9金属酶的铜绿假单胞菌,ERIC-PCR对产酶株进行同源性分析.结果 从2007年10月到2008年11月收集广州7家大型医院的269株铜绿假单胞菌泛耐株,共检测出携带IMP-9金属酶的铜绿假单胞菌10株,均来自同一医院.ERIC-PCR检测,10株均为同一基因型的克隆株,但与前期曾出现的产IMP-9金属酶克隆株无同源性.结论 产IMP-9金属酶铜绿假单胞菌的院内克隆传播仍然存在,而医院间的传播在本次调查中没有发现.因此,仍需继续加强对铜绿假单胞菌定植患者的监控和加强院内感染的管理控制.

Abstract:Objective To screen IMP-9 type metalloenzyme producing strains from pan-drug resistant Pseudomonas aeruginosa collected from seven hospitals in Guangzhou from Oct 2007 to Nov 2008,analyzing the clonal relationship between the strains and one epidemic strain spread in the same hospital.Methods The Q-PCR and analysis of DNA sequence were used to screen out Pseudomonas aeruginosa producing IMP-9 metalloenzyme,then ERIC-PCR were used to identify to the genotypes of the strains.Results There were 10 strains produced IMP-9 metalloenzyme in total 269 pan-drug resistant Pseudomonas aeruginosa,and the 10 strains isolated from the same hospital,which occurred an outbreak of IMP-9 producing strain of P aeruginosa during 2005 to 2007.Surprisingly,the 10 strains belonging to one clone,have no relationship with the epidemic strain in 2005.Conclusions The epidemic clone of pan-drug resistant Pseudomonas aeruginosa presented in the 2005,haven't been found this time,but there was a new clonal dissemination in the same hospital.It was important to manger the patients colonized with epidemic clone only to prevent transmission of clone.
