系统化整体化护理对断指再植术后手功能恢复的影响

目的 探讨断指再植术后系统化整体化护理措施对手功能恢复的影响。方法 将1998年11月～2000午5月行断指再植术的病人随机分组,实验组146例206指,术后进行系统化整体化护理及早期功能锻炼;对照组130例186指,术后行传统护理方法。两组均于术后6个月随访,进行功能评定,并统计术后6个月恢复原工作的病例数,进行统计学分析。结果 实验组优139指,良53指,优良率93.3％,恢复原工作133例(91.1％);对照组优86指,良38指,优良率66.7％,恢复原工作69例(53.3％)。两组手功能恢复比较x~2=4.40,P<0.05,两组恢复原工作率比较x~2=5.07,P<0.05,以上两项比较差异均有统计学意义。结论 断指再植术后系统化整体化护理措施及早期功能锻炼,能够预防肌腱粘连、关节僵直及疤痕挛缩等并发症,促进手功能恢复。

Effect of Systematic and Holistic Nursing on the Functional Recovery of Hand with Replantation of Severed Finger

Objective To explore the effect of systematic and holistic nursing on the functional recovery of hand with eplantation ot severed finger. Methods The cases receiving replantation of severed finger from November, 1998 to May,2000 were randomly divided into experimental group and control group. In experimental group,146 cases(206 fingers)received sys-temtic and holistic nursing and early functional exercise after the operation; while in control group,130 cases(186 fingers)re-ceived traditional nursing. After 6 months ,the hand function of all the cases were measured. Statistic analysis of the data of the two groups was made. Results In experimental group:excellent 139 fingers,good 53 fingers ,the total excellent and good rate was 93. 3% ,the cases returned former work were 133(91.1%). In control group excellent 86 fingers,good 38 fingers,the total excellent and good rate was 66. 7% ,the cases returned former work were 69(53. 3%). The excellent and good fingers of the experimental group and the cases returned former work were significantly higer than those of the control group(X2 = 4. 40, P<0. 05;X2 = 5. 07,P<0. 05). Conclusion Systematic and holistic nursing and early functional exercise can avoid the tendon adhesion and joint stiff and scar contraction, it can improve the functional recovery of the hand with replantation of severed finger.
