新生儿窒息胎盘的组织病理学与形态计量学观察

摘    要:目的:初步探讨新生儿窒息与其组织定量改变的关系。方法:应用形态测量技术对27例轻度窒息新生儿(轻度窒息组),23例重度窒息新生儿(重度窒息组)及20例正常新生儿(对照组)的胎盘绒毛进行形态计量学检测。结果 (1)重度窒息组胎盘绒毛具有合体细胞结节、细胞滋养细胞增生的绒毛数显著高于正常组(P<0.05);具有血管合体细胞形态的绒毛数明显低于对照组(P<0.05);(2)重度窒息组绒毛毛细血管面密度低于对照组(P<0.05);绒毛面密度与对照组比无显著差(P>0.05)。而在轻度窒息组具有合体细胞结节、细胞滋养细胞增生及血管合体细胞膜的绒毛数及绒毛血管面密度与对照组比均无显著性区别(P>0.05)。结论 胎盘组织定量学的改变可能是引起新生儿重度窒息的重要原因。

Abstract:Objective: To study the relationship between the Asphyxia neonatorum and its quantitative changes of placental parenchyma. Methods: The placental parenchymas of 27 cases with mild asphyrial neonates (MAN),23 cases with severe asphyxial neonates (SAN), and 20 cases of normal neonates (NN) were quantitatively analyzed by image analysis.Results:(1)The number of the placental villus with the syncytial knots cytotrophoblasts in SAN group was bigger than that in NN group ( P <0.05).And the number of the placental villus with the vasculosyncytial membranes in SAN group was significantly smaller than that in NN group( P <0.05).(2)The area density of the villous fetal capillary in SAN group was significantly reduced compared with that in the NA group( P <0.05). There were no significant differences of numbers of the placental villus in the syncytial hnots, cytotrophoblasts, vasculosyncytial membranes, and the area density of the villous fetal capillary between the MAN and NA groups.Conclusion: The significant quantitative changes in the placental parenchyma may be an important factor causing the SAN.
