经肩胛下胸腔入路的上胸椎肿瘤切除及脊柱稳定性重建术

目的：探索上胸椎肿瘤外科治疗的新途径。方法：采用经肩胛下胸腔入路行上胸椎肿瘤切除，脊髓减压，钢筋骨水泥重建脊柱稳定性。结果：本组6例中无手术死亡及纵膈内重要血管，神经及胸导管损伤所致的并发症。术后随访3－13个月，6例患者肌力损害及疼痛均有不同程度的改善。结论：经肩胛下胸腔入路切除上胸椎肿瘤，手术视野开阔，病变节段显露清楚，内固定操作简易，并发症少，是处理上胸椎病变的又一较理想的入路。

Excision of Upper Thoracic Spine Tumor and Reconstruction of Stability of Spine through Transthoracic Posterolateral Approach

Objective: To explore a new approach to upper thoracic spine tumor. Methods: Transthoracic approach was used to commit excision of upper thoracic spine tumor, decompression of spinal cord and reconstruction of stability of spine. Results: There were no mortality or serious complication as results of injuries of vessels, nerves or thoracic duct in mediastina. The follow up period ranged from 3 13 months. The impairment of muscle strength and pain were ameliorated in all of 6 patients. Conclusion: Process of upper spine was exposed very clearly and the operation field was widened with this approach. It was especially suit for internal fixation of upper thoracic spine with few complications. It may be an ideal approach to treat disorders of the lesion of upper thoracic spine.
