单纯胆囊切除术与根治性切除术治疗Ⅱ期胆囊癌远期疗效分析

目的分析单纯胆囊切除术与根治性切除术治疗Ⅱ期胆囊癌远期疗效差异。方法选择2012年11月-2013年11月在本院接受治疗的Ⅱ期胆囊癌患者作为研究对象，随机分为接受单纯胆囊切除术的对照组患者及接受根治性切除术的观察组患者各43例。随访5年分析二组患者的生存时间及各年生存率、生活质量评分等差异。结果①接受手术治疗后，观察组患者的中位生存期明显长于对照组患者，且其术后1年、3年及5年的累积生存率也大幅高于对照组患者（P〈0．05）；②接受手术治疗后，观察组患者的远期平均生活质量评分明显高于对照组患者，且生活质量评分优良率明显高于对照组患者（P〈0．05）。结论根治性胆囊切除术可以更为有效的延长胆囊癌患者的生存时间，提高远期生存率并改善其生活质量，较单纯胆囊切除术具有明显优势。

LONG - TERM EFFECT OF SIMPLE CHOLECYSTECTOMY AND RADICAL RESECTION ON GALL-BLADDER CANCER PATIENTS

Objective To analyze the difference of long-term effect of simple cholecystectomy and radical resection on gallbladder cancer. Methods Author chose patients with gallbladder cancer of Phase Ⅱ between November 2012 to November 2013 as research subjects, randomly assigned to receive simple cholecystectomy as control group and radical resection as observation group, each with 43 pa-tients. Five-year survival rate and median survival time, quality of life scores were recorded. Results After surgery, the median survival of patients in the observation group was significantly longer than control group patients, and postoperative 1 - year, 3-year and 5- year cumulative survival rate were significantly higher than control group patients （P〈0.05）. After surgery, the life quality scores of observation group were significantly higher than control group patients, and the excellent rate of life quality scores were significantly higher than control group patients （P〈0. 05）. Conclusions Radical cholecystectomy can be more effective on prolonging the survival time of patients with gallbladder cancer of Phase and improve long-term survival rate and their quality of life, has obvious advanta- ges c ompared with simple cholecystectomy.
