胰腺癌区域化疗和全身化疗时5-Fu在门、体静脉血中的动态分布及差异

摘    要:目的 为胰腺癌区域化疗提供临床实验依据。方法 15例手术不能切除的胰腺癌患者行转流术后,分别经区域动脉或体静脉快速推注5-Fu1000mg,用反相高效液相色谱法测定门、体静脉血中5-Fu的浓度,也显著高于同期全身化疗组门静脉血中5-Fu的浓度;而体静脉血中的药物浓度略低于同期全身化疗组。结论 胰腺癌区域化疗可提高胰腺区域的化疗药物浓度,而体静脉血中的化疗药物浓度可保持在较低水平,从而增强胰腺癌化疗的效果并相对减轻药物的毒性反应。

Dynamic distribution and comparsion of 5-Fu in portal vein and systemic vein in patients with pancreatic cancer receiving regional or systemic chemotherapy
Abstract:Objective To study dynamic distribution of 5 Fu in the portal vein and systemic vein of patients with pancreatic cancer after giving administration of intraarterial and intravenousl 5 Fu. Methods 15 patients with inoperable pancreatic cancer were divided into two groups. In the experimental group (9 patients), a catheter was inserted into the gastroduodenal artery or right gastroepiploic artery. A bolus 5 Fu at the dose of 1?000 mg was infused through the catheter. In the control group (6 patients), a bolus 5 Fu at the same dose was infused through the limb vein. Blood samples were obtained from the portal vein and limb vein for up 60 minutes (10, 20, 30, 60 minutes respectively) after the initiation of drug infusion in both groups. The serum fluorouracil concentrations were determined by reversed phase HPLC method. Results In the experiment group, the concentration of 5 Fu in the portal vein was significantly higher than that in the systemic vein (limb vein) at 10, 20 and 30 minutes respectively; but in the control group, the differences were not significant. The concentration of 5 Fu in the portal vein in the experiment group was significantly higher than that in the control group at 10, 20 and 30 minutes respectively, while the concentration of 5 Fu in the systemic vein was similar in both groups at all time points. ConclusionThese results indicate that regional chemotherapy can produce a significant increase in pancreatic area drug exposure and may keep the low concentration of antineoplasm drug in the systemic vein, which will be helpful to the treatment of pancreatic cancer.
