摘    要:目的 通过对反复自然流产(RSA)患者和正常早孕妇女绒毛、蜕膜中生存素(survivin)及血管内皮生长因子(VEGF)的测定,探讨RSA患者的流产病因和胎盘的生长机制.方法 采用免疫组化法检测40例RSA患者及20例对照组的绒毛和蜕膜组织中survivin、VEGF含量.结果 Survivin、VEGF在RSA患者孕早期绒毛的表达均显著高于对照组(P〈0.05),两组绒毛中survivin与VEGF的表达呈正相关 (rs=0.268,P〈0.05);蜕膜中二者的表达无相关性(rs=0.091,P〉0.05).结论 在胚胎发育和胎盘形成过程中VEGF可能通过上调survivin,促进细胞增殖、血管形成和抗细胞凋亡的作用,参与胚胎着床、胎盘生长和浸润.VEGF、survivin的高表达可能导致了流产的发生.

Abstract:Objective: To measure the expression of survivin and vascular endothelial growth factor (VEGF) in villus and decidua in women with recurrent spontaneous abortion (RSA) and normal early pregnant women, in order to probe the possible mechanism of RSA and embryo implantation and placentation. Methods: Villus and decidual tissues were collected from 40 RSA women with embryo growth arrest and 20 age and gestational week-matched normal pregnant women who sought an artificial abortion. The expression of survivin and VEGF was detected by immunohistochemistry. Results: The expression of either survivin or VEGF in first-trimester villus tissues in women with RSA was statistically higher than that in normal pregnant women (P〈0.05), while the expression of survivin or VEGF in decidua showed no significant difference between two groups (P〉0. 05). For both women with RSA and normal pregnant women, the expression of VEGF was positively correlated with the expression of survivin in villi (rs=0. 268, P〈0.05) but not in deciduas (rs =0. 091, P〉0.05). Conclusions: The up-expression of survivin and VEGF in villous tissues may lead to pregnancy loss.
