外耳道胆脂瘤的CT分期

目的探讨外耳道胆脂瘤(external auditory canal cholesteatoma,EACC)的CT分期方法。方法对66例手术治疗的EACC病例进行回顾性分析,根据颞骨CT结果将EACC分为四期:EACC局限于外耳道内且无明显骨质破坏为Ⅰ期;EACC破坏外耳道骨质,但未侵犯鼓室或乳突为Ⅱ期;EACC侵范鼓室和或乳突为Ⅲ期;EACC侵犯至颞骨外部结构为Ⅳ期。并将CT分期结果与术中所见结果进行比较。结果 66例EACC中,Ⅰ期13例,Ⅱ期36例,Ⅲ期16例,Ⅳ期1例。EACC的CT分期与术中所见的总符合率为96.9%(62/66)。结论 EACC的CT分期方法简单、可靠,有助于选择正确的EACC手术方式。

Staging of external auditory canal cholesteatoma by computed tomography

Objective To determine the staging of external auditory canal cholesteatoma(EACC) by the computed tomography findings.Methods A total of 66 patients with EACC who underwent surgical therapy were retrospectively evaluated.According to the results of temporal bone computed tomography,we classified EACC into four stages: Stage I indicated that the EACC lesion was limited to the external auditory canal without a defective bone canal.Stage II indicated that the EACC lesion created a defect of the external auditory canal.Stage III indicated that the EACC lesion invaded the middle ear and or the mastoid cells.Stage IV indicated that the EACC lesion was beyond the temporal bone.We also compared the staging results of CT with the observing results during operation.Results Of these 66 EACC cases,the number of stage I,II,III,and IV cases was 13,36,16,and 1,respectively.The general coincidence rate of the CT Staging and operation observing results was 96.9%(62/66).Conclusions Our staging system of EACC due to the computed tomography findings is simple and reliable,which can be applied to the appropriate surgical method of EACC.
