摘    要:目的:总结肺结核大咯血急症手术的麻醉处理方法及术中管理经验,以期提高此类手术的麻醉水平.方法:对我院2006年1月至2011年1月收治的92例肺结核大咯血急症手术患者实施麻醉,并详细记录术前准备、麻醉诱导、术中呼吸、循环管理及术后苏醒等经过.结果:本组病例均顺利完成手术,87例患者在手术室拔除气管插管后安返病房;5例患者因呼吸恢复不佳,带管转入ICU.结论:肺结核大咯血的麻醉处理关键是:双腔气管插管的位置准确,加强呼吸道管理,保证健肺通气;维持循环稳定,保持麻醉平稳.

Abstract:Objective: Summary of the anesthesia methods and intraoperative management experience in the emergency operation for massive hemoptysis of pulmonary tuberculosis,in order to improve the operation level of anesthesia.Methods: In our hospital in January,2006 to January,2011,92 patients with emergency operation for massive hemoptysis of pulmonary tuberculosis were anesthetized,and we made a detailed record of preoperative preparation,induction of anesthesia,intraoperative respiratory,circulation management and postoperative recovery.Results: The operations of all the cases were successfully completed,87 patients went back to ward after operation room extubation;Five cases due to poor recovery with breathing were transfered to ICU.Conclusion: The key of anesthesia treatment to hemoptysis of pulmonary tuberculosis: double lumen endotracheal intubation tube positions are accurate,the airway management should be strengthened,to ensure the healthy lung ventilation,to maintain circulatory stability and smooth anesthesia.
