慢性阻塞性肺疾病患者血小板活化指标的变化及其意义

目的观察慢性阻塞性肺疾病（Chronic obstructive pulmonary disease,COPD）患者血浆血小板活化指标CD62P、CD63、CD41、CD61,探讨血小板聚集功能改变在COPD发生发展中的作用及临床意义。方法采用流式细胞术测定68例慢性阻塞性肺疾病患者血小板活化指标CD62P、CD63、CD41、CD61治疗前后的改变及进行比较,并与36例健康对照组进行比较。结果急性期组病人血浆的CD62P、CD63显著高于对照组及缓解期组,缓解期组病人血浆的CD62p、CD63显著高于对照组;急性期组和缓解期组病人血浆的CD41、CD61比较差异均无统计学意义。直线相关分析表明COPD急性期组动脉血氧分压与血浆CD62P水平间呈显著负相关,动脉血二氧化碳分压与血浆CD62P水平间呈显著正相关。结论慢性阻塞性肺疾病患者的血小板活化程度增高,尤其在急性期的患者的血小板活化增高更加显著;推测血小板活化程度增高在COPD发生发展中起重要作用。

Relationship between platelet activation and chronic obstructive pulmonary disease

Objective To observe the platelet activation in patients with chronic obstructive pulmonary disease（COPD）.Methods The changes of CD62P,CD63,CD4,CD61 in platelets which reflects the platelet activity were measured in 68 patients with COPD and 36 healthy subjects（controls）,and then the results were compared between two groups.Results Plasma levels of CD62P and CD63 in COPD patients was statistically higher than those in controls,especially in acute exacerbation stage.Conclusion There is an abnormal platelet activation in COPD patients,especially when the condition worsens.Activated platelets may play the major role in the development of COPD.
