前臂桡动脉逆行岛状皮瓣修复手腕部软组织缺损

［ 背景］ 手和腕部软组织缺损修复方法多种多样．［ 病例报告］ 用前臂桡动脉逆行皮瓣治疗３ 例手腕部软组织缺损．软组织缺损面积最大１０－０ｃｍ ×７－０ｃｍ ，最小６－０ｃｍ ×５－０ｃｍ ．腕骨和肌腱外露，瘢痕挛缩影响手的功能．术前用多普勒血流仪探查前臂血管、掌浅弓和掌深弓．皮瓣游离后夹闭近侧端桡动脉，观察皮瓣和手指末梢血运１０ｍｉｎ，如无异常，结扎切断近心侧桡动脉逆行转移皮瓣修复手腕部软组织缺损．３ 例皮瓣血运均良好．［ 讨论］ 前臂桡动脉逆行皮瓣适合修复手腕部软组织缺损

The repair of soft tissue defect of hand and wrist by forearm skin flap

BACKGROUND The method of repairing the soft tissue defect of hand and wrist is diverse. CASE REPORT 3 patients with soft tissue defect of the hand and wrist were treated by the forearm radial artery skin flaps which were distally based.The largest area of soft tissue defect was 10 0cm×7 0cm and the smallest was 6 0cm×5 0cm.Bones and tendons were exposed,the scar of the hand affected the hand functions.Before the operation, forearm vessels,superficial palmar arch and deep palmar arch would be studied with Doppler's. During the operation, after the flap was raised, the proximal section of radial artery would be obstructed temporarily by the hemostatic forceps for ten minutes to observe the blood supply of the flap and the hand. If there is no problem in blood supply, the flap is transplanted distally to repair the soft tissue defect of the hand. All flaps were good in blood supply. DISCUSSION The distally based forearm radial artery flaps are optimal in repairing the soft tissue defects of the hand and wrist.
