颅内后循环动脉瘤和动静脉畸形的影像诊断

摘    要:目的:研究颅内后循环动脉瘤和动静脉畸形(AVM)的影像学表现。方法:26例后循环动脉瘤和动静脉畸形患者均行常规头颅CT及DSA检查,其中10例患者行头颅MR检查。结果:后循环动脉瘤19例中基底动脉瘤5例,椎动脉瘤7例,小脑下后动脉瘤7例。CT可显示蛛网膜下腔出血及小脑出血。DSA可明确显示畸形血管部位及大小。结论:常规头颅CT和MR可有效显示后循环动脉瘤和动静脉畸形的颅内出血情况及较大的动脉瘤和畸形血管团。DSA仍是诊断颅内后循环动脉瘤和动静脉畸形的金标准。

Abstract:Objective To evaluate the image manifestations of intracranial aneurysm and arteriovenous of posterior circulation. Methods 26 cases of intracranial aneurysm and arteriovenous of posterior circulation were checked by plain CT scan and cerebral angiography. MR scan was performed in 10 cases. Results The main manifestations of intracranial aneurysm and arteriovenous of posterior circulation on CT were subarachnoid hemorrhage and cerebral hemorrhage. Among all 19 cases of intracranial aneurysm of posterior circulation,5 cases located at basilar artery,7 cases were located at vertebral artery,7 cases located at posterior inferior cerebellar artery.In 7 cases of posterior fossa AVM,6 cases were cerebellum AVM,1 case were medullary AVM. Conclusion Both CT and MR can reveal intracranial hemorrhage and big aneurysm or AVM effectively. DSA is the gold stand of diagnosing intracranial aneurysm and arteriovenous of posterior circulation.
