自体表皮移植术联合小剂量糖皮质激素口服治疗进展期白癜风临床疗效观察

目的观察自体表皮移植术联合小剂量糖皮质激素口服治疗进展期白癜风临床疗效。方法以自体表皮移植术联合泼尼松口服治疗51例进展期白癜风,表皮移植结束3月后进行疗效观察,并对寻常型及节段型两组的疗效进行比较。结果51例进展期白癜风患者共移植皮片337片,总有效率86%。40例寻常型患者共移植241片,总有效率83%。11例节段型患者共移植96片,总有效率96%。寻常型组及节段型组总有效率比较差异有统计学意义（x~2=9.398,P〈0.01）。40例寻常型患者中31例（77.5%）患者转为稳定期,11例节段型患者中9例（81.8%）患者转为稳定期。2例寻常患者在供皮区出现同形反应。结论自体表皮移植术联合小剂量糖皮质激素口服能迅速控制病情,使白癜风进展期转变为稳定期,提高自体表皮移植治疗白癜风的成功率。

Autologous epidermal grafting combined with low dose steroids for progressive vitiligo

Objective To evaluate the efficacy and safety of autologous epidermal grafting combined with low dose steroids in treatment of progressive vitiligo.Methods Fifty one patients with progressive vitiligo were treated with the combination therapy.Low dose prednisone was administrated orally before and after autologous epidermal grafting.Effectiveness was evaluated 3 months after the transplantation. Results The overall effective rate in segmental vitiligo group was 96%(96 pieces in 11 cases) and was significantly higher than that of vitiligo vulgaris group(83%,241 pieces in 40 cases).Thirty one patients(78%) achieved stable in vulgaris group.9 patients(82%) achieved stable in segmental group; 2 patients developed isomorphism in donor sites.Conclusion Autologous epidermal grafting combined with low dose oral steroids can raise the effective rate in patients with progressive vitiligo.
