正常脑脊液状态下FLAIR序列脑沟高信号的MRI研究

摘    要:目的 探讨无明显脑脊液异常的病人在FLAIR图像上出现脑沟高信号的机制和特点。方法 对 3 0 0例无明显脑脊液异常的病人在头部FLAIR图像上是否存在脑沟高信号进行前瞻性MRI筛选 ,对 3 0例FLAIR图像上存在脑沟高信号的病例在分布、信号及有无增强等方面进行评估。结果  2 85个病人中 3 0例 (10 .5 % )表现为FLAIR图像上脑沟高信号 ,其中 16例 (5 .6% )占位性病变 ,14例 (4 .9% )血管性疾病。FLAIR图像上脑沟高信号通常与混浊脑脊液征 (在非增强T1WI像上脑脊液信号轻度增加 ,在T2WI像上脑脊液信号轻度减低 ) (66.7% )和异常对比增强 (95 .0 % )有关。结论 研究表明FLAIR图像上脑沟高信号可以发生于无明显脑脊液异常的病人。它通常与占位性病变、脑血管病、异常血管增强和混浊脑脊液有关

Abstract:Objective To investigate the clinical history and MR findings associated with sulcal hyperintensity on FLAIR images in patients without apparent CSF abnormality. Methods Three hundred consecutive MR imaging examinations were prospectively screened for patients with sulcal on FLAIR images of head. The distribution of sulcal hyperintensity on FLAIR images and associated abnormal MR findings were evaluated. Results Thirty (10.5%) of the 285 patients had sulcal hyperintensity associated with 16 cases masses (5.6%) and 14 cases vascular abnormalities (4.9%). Sulcal hyperintensity was frequently associated with the dirty CSF sign (66.7%) and abnormal contrast enhancement (95.0%). Conclusion Our study shows that sulcal hyperintensity on FLAIR imaging can occur in patients without apparent CSF abnormality. It is frequently associated with mass, vascular disease, abnormal vascular enhancement and dirty CSF sign.
