眼动脉瘤的诊断和栓塞治疗

目的：探讨颈内动脉眼动脉瘤的诊断和栓塞治疗。方法：18例患行CT或MRI检查，并均经DSA确诊。13例采用电解可脱卸弹簧圈（Gugliemi detachable coil,GDC)栓塞治疗，5例因动脉瘤过大采用支架植入聚合GDC栓塞治疗。结果：GDC栓塞治疗13例，达致密栓塞9例，大部栓塞4例；支架植入联合GDC栓塞治疗5例，致密栓塞4例，大部栓塞1例，12例行DSA随访6个月，均未见动脉瘤复发或支架内膜过度增生导致载瘤动脉狭窄。结论：眼动脉瘤的栓塞治疗是有效的，大型眼动脉瘤CT、MRI有典型特征，采用支架植入联合GDC栓塞可提高其致密栓塞率。

Diagnosis and endovascular treatment of ophthalmic aneurysms

Objective:To evaluate the diagnosis and endovascular treatment of ophthalmic aneurysms.Methods: Eighteen patients were checked by CT or MRI,and were diagnosed with DSA.Thirteen cases of aneurysms were embolized only with Gugliemi detachable coils(GDC),and 5 were treated with stent placement and subsequent GDC.Results: In the thirteen aneurysms treated only with GDC,total embolization was achieved in 9 and partial embolization in 4.After stent placement and coiling,4 aneurysms were densely packed and another 1 was partially packed.Conclusion: Endovascular treatment of ophthalmic aneurysms is effective and safe.CT and MRI have a special show of large and giant aneurysms.Combined stenting and coiling is an alternative method for the treatment of large and giant ophthalmic artery aneurysm with the preservation of parent artery.
