数码图像内镜系统在颅底外科的应用

摘    要:目的:进一步探讨内窥镜技术在颅底外科的应用价值。方法:在应用常规内窥镜手术进行颅底手术的基础上,根据不同的患者及病变,结合使用特种内窥镜及数码图像技术辅助进行颅底手术。结果:进行颅底手术共47 例,年龄最小为2 岁半,最大72 岁,其中脑脊液鼻漏探查修补13 例,全部成功;视神经减压5 例,3 例视力改善。颅内外沟通瘤联合手术7 例,4 例痊愈,1 例好转,2 例失访。其它22 例不同的病例也获得较好效果。结论:内窥镜数码图像系统使颅底手术操作条件得到进一步改善。由于视角大,图像清晰,手术可做得更加精细彻底,创伤小,部分病例可明显提高疗效。

Abstract:Objective:To explore the values of digital image endoscopic system in skull base surgery. Methods:According to different cases, the skull base sugeries were performed under the assistance of routine or special endoscopic digital image system.Results:47 cases of skull base surgeries were performed.The youngest patient was two and half years old,and the oldest was 72. 13 cases of cerebrospinal fluid (CSF) fistulae were repaired successfully under endoscope. 5 cases of endoscopic optic nerve decompression were performed for traumatic optic neuropathy, 3 of them got improved vision. 7 cases of transskull tumor were resected by the assistance of endoscopic technique.4 of them were recovered and one got improved,and the others loss contact. 22 cases of different skull base surgeries also resulted well.By means of needle endoscope,the examination and operation could be performed in narrow cavities and sinus.With microendoscope,minor disorder could be detected more clearly.Different parts of cavity and sinus could be observed by means of different visual angle endoscope.Conclusion:With wide visual angle and high resolution, the digital image endoscope system allows skull base surgery to be performed more perfectly and radically with less invade and better result.
