Abstract:Objective To investigate the effectiveness of clinical application of an extracorporeal assisted targeting device for distal locking nails of interlocking intramedullary nails of the tibia. Methods Retrospective analysis was performed on the data of 54 patients with tibial shaft fractures treated with interlocking intramedullary nails in the third department of traumatic Bone in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine, Hebei Province from July 2018 to May 2020. The patients were divided into two groups according to the auxiliary application of an extracorporeal assisted targeting device for distal locking nails of interlocking intramedullary nails of the tibia. The treatment group included 29 patients, using the distal locking of the tibial interlocking intramedullary nail with the aid of an extracorporeal targeting device for the distal locking of the tibial interlocking intramedullary nail. There were 20 males and 9 females, with an average age of (49.4±11.9) years. There were 11 cases on the left side and 18 cases on the right side. AO/OTA classification of fracture: 12 cases of type 42-A, 10 cases of type 42-B and 7 cases of type 42-C. The control group included 25 patients, only the internal medullary nail manufacturer''s own aiming device was used in the distal locking process. There were 18 males and 7 females, with an average age of (49.6±12.7) years old. There were 9 cases on the left and 16 cases on the right. AO/OTA classification of fracture: 10 cases of type 42-A, 7 cases of type 42-B, and 8 cases of type 42-C. The success rate of the first distal locking, the time of the distal positioning locking, the operation time, the number of intraoperative X-ray fluoroscopy of the distal locking, the intraoperative blood loss, and the postoperative incision infection rate of the two groups were compared. Results All the 54 patients successfully completed the operation, and there were no serious complications during and after operation. The time of positioning distal locking, operation time, intraoperative X-ray fluoroscopy times of distal locking and intraoperative blood loss of the treatment group were significantly less than those of control group, with statistically significant differences (P<0.05). The success rate of distal one-time locking in the treatment group was significantly higher than that in the control group, and the difference was statistically significant (P<0.05). There was no significant difference in postoperative incision infection rate between the two groups (P>0.05). Conclusion The extracorporeal assisted targeting device for distal locking nails of interlocking intramedullary nails of the tibia can significantly shorten the time of distal tibial intramedullary nail placement, and reduce the number of intraoperative X-ray fluoroscopy and intraoperative blood loss of the distal tibial nail.
