多层螺旋CT三维重建在颌面部骨折诊断中的价值

摘    要:目的探讨多层螺旋CT三维重建在颌面部骨折诊断中的价值。方法对2009年4月至2013年4月来中山市小榄人民医院住院诊治的84例颌面部骨折患者进行多层螺旋CT扫描,并进行多平面重建(MPR)及容积成像(VR),分析其骨折情况。结果84例患者共发现骨折线392条,MPR可发现骨折线392条(100%),常规CT扫描图像发现骨折线328条(83.7%),其中上颌骨、鼻骨、筛骨、蝶骨骨折的显示率显著低于MPR图像(P〈0.05),眶骨、颧骨和下颌骨的显示率比较差异无统计学意义(P〉0.05):VR共显示294条骨折线(75.0%),其中上颌骨、眶骨、鼻骨、筛骨、蝶骨骨折显示率显著低于MPR图像(P〈0.05),颧骨和下颌骨的显示率比较差异无统计学意义(P〉0.05)。结论多层螺旋CT三维重建能够全面评估骨折情况,提高骨折的诊断准确率,其在颌面部骨折的诊断中具有重要的价值。

Abstract:Objective To explore the value of multislice spiral CT three-dimensional reconstruction in diagnosis of maxillofacial bone fractures. Methods Eighty-four hospitalized patients with maxillofacial bone fi'actures in the department of stomatology from Zhongshan Xiaolan People's Hospital during Apirl 2009 to Apirl 2013 underwent muhisliee spiral CTimuhi planar reformation (MPR) and volume rendering(VR). The image data was analyzed. Results A total of 392 fracture lines were found in the 84 patients. Total of 392 fracture lines were found hy MPR images ( 100% ) ,328 fracture lines were found by CT scanned images(83.7% ), the show rate of fracture of maxilla, nasal bone, ethmoid and sphenoid were lower than that of MPR images( P 〈 0.05 ) , there was no difference between orbital hone,zygomatic, and mandible on show rate ( P 〉 0.05 ). 294 fracture lines were showed by VR (75.0%) , the show rate of fracture of maxillary,orbital bone, nasal, ethmoid and sphenoid bone were lower than that of M PR images( P 〈 0.05 ) ,there were no differences between malar and mandibular on show rate( P 〉 0.05 ). Conclusion Muhisliee spiral CT three-dimensional reconfftruetion can find the fractures totally and it can improve the accuracy rate of diagnosis,which has a significant value for diagnosis of maxillofacial bone fractures.
