74例大细胞肺癌的影像特点

目的探讨大细胞肺癌的影像特点。方法回顾性分析74例大细胞肺癌患者的X线片表现，其中36例行cT检查。结果74例大细胞肺癌的X线片表现为团块状影或结节影68例（91．9％），周围型者44例（59．5％），50例（67．6％）位于上叶，约半数患者可见分叶，平均直径约6．5cm，支气管气象、空洞、钙化少见；36例CT表现与X线片表现相似，周围可见毛玻璃密度影、肺气肿，23例（62．9％）伴有淋巴结大，约半数患者病灶内可见坏死，增强扫描不均匀强化最多见（50％）。结论影像学检查有助于大细胞肺癌的诊断。

The Imaging Findings of 74 Cases with Lager Cell Lung Cancer

Objective To investigate the imaging features of large cell lung cancer（ LCLC ）. Methods Imaging findings of 74 cases with LCLC proved by pathology were reviewed retrospectively. All patients received X-ray chest film and 36 of them received CT scan. Results 68 lesions（91.9% ） were presented as a mass or nodule in X-ray film. 44 lesions（59.5% ） were peripheral ,50 lesions（67.6% ）located in the upper lobes and about half cases were lobu｜ated. The average diameter of 74 lesions was 6.5 cm. Air bronehogram, cavity and calcification were not often seen. The CT findings of 36 cases were similar to the X-ray findings. Ground-glass opacity and emphysema was found on the peripheral of the lesion. Hilar or mediastinal lymph nodes metastasis were seen in 23 eases（62.9% ）. Necrosis was seen in about half of the cases. Inhomogencous enhancement was mostly（50% ） seen in enhanced CT scan after contrast. Conclusion The imaging findings of LCLC are characteristic to some extent.
