腹腔镜胃癌根治术的临床应用

目的：探讨腹腔镜胃癌根治术的可行性和效果。方法：14例胃癌患者行腹腔镜胃癌根治术,其中根治性远侧胃次全切除术12例;根治性全胃切除术2例。结果：除1例中转开腹,1例改行经胸食管空肠吻合外,其余全部成功进行腹腔镜胃癌根治术。手术用时平均260 min（210-350 min）,出血量平均180 mL（80-300 mL）,清扫淋巴结平均21枚（13-27枚）,肛门排气时间平均2.5 d（1-4 d）。结论：腹腔镜胃癌根治术安全可行,能够达到开腹手术相当的根治效果。

The Application of Laparoscopic Radical Gastrectomy

Objective:To explore the feasibility,method and effect of laparoscopic radical gastrectomy.Methods:In a total of 14 cases of gastric cancer,there were 12 cases receiving radical distal gastrectomy,2 receiving total gastrectomy.Results:Twelve cases underwent laparoscopic radical gastrectomy successfully,but 1 case converted to open surgery,1 cases converted to open chest for esophagojunostomy.The mean operation time was 260 min(210~350 min),the mean volume of blood loss was 180 mL(80~300 mL),the mean numbers of harvested lymph nodes were 21(13~27),the mean time was 2.5 d(1~4 d)for gastrointestinal function recovery.The short-term efficiency was obvious.Conclusion:Laparoscopic radical gastrectomy is a feasible,safe and minimally invasive treatment and can achieve the same outcome as the open operation.
