超声多普勒测定糖尿病孕妇脐动脉,子宫动脉血流速波的研究

摘    要:本文利用超声多普勒,从孕20-40周,每隔2周连续测定一组糖尿病孕妇(n=21)脐动脉,子宫动脉的血流速波,并与正常孕妇组(n=20)对照,以探讨母体糖尿病对胎儿胎盘循环的影响。结果发现,出生体重符合孕龄(AGA)糖尿病孕妇(n=15),脐动脉,子宫动脉S/D比值与正常孕妇组比较,无明显差异。而出生体重大于孕龄(LGA)糖尿病孕妇(n=6)的脐动脉,子宫动脉的S/D比值在孕30周后出现高值,而且,S/D比值升高的时间与糖尿病LGA儿的生长加速阶段相一致。提示,LGA糖尿病孕妇S/D比值的升高可能与糖尿病LGA儿在生长加速阶段的血流需要量的增加有关。

Abstract:Effects of diabetes mellitus (DM) on fetal-Placental circulation were evaluated with Doppler velocimetry in umbilical artery (Um) and maternal uterine artery (Ut) in pregnancy complicated with DM. The systolic diastolic ratio (S/D) was obtained from blood flow wave form of Um and Ut every 2 weeks from 20 weeks of gestation untill delivery in 21 cases with DM. Both Um and Ut S/ D ratio decreased with the advancement of gestational age in normal control group (n=20) and DM cases without large for gestational age (AGA) infant (n=15). In contrast, these values remained elevated in DM cases with LGA (n=6) from 30 weeks of gestation untill term in which the growth of LGA fetus accelerates. The S/D of both Um and Ut in DM with LGA obtained during 36 to 40 weeks of gestation were significantly higher than those of normal control and DM with appropriate for gestational age infant. It is suggested that the relative increase in vascular resistance in feto placental circulation due to the accelerated fetal growth in DM may account for the elevation of S/D.
