锁定加压钢板与人工股骨头置换治疗高龄不稳定型股骨粗隆间骨折的比较

摘    要:目的:对采用人工股骨头置换与锁定钢板内固定治疗高龄不稳定型股骨粗隆间骨折的患者进行分析比较,探讨其临床疗效。方法:回顾性分析2004年2月~2009年5月间接受锁定钢板内固定、人工股骨头置换治疗且有完整资料的76例高龄股骨粗隆间骨折的患者。其中锁定钢板内固定组42例,人工股骨头置换组34例;均为不稳定型骨折。比较2种治疗方法在手术时间长短、切口大小、术中出血量和术后引流量、术后Harris关节功能评分、内科并发症及Barthel生活指数等方面的指标。结果:锁定钢板组和人工股骨头置换组的平均随访时间分别为19.2、17.5个月。锁定钢板组具有手术时间短、切口小术中出血少等优点(P〈0.05),在术后关节功能的恢复、内科并发症及Barthel生活指数二者没有明显的区别(P〉0.05)。结论:人工股骨头置换和锁定钢板均为治疗高龄不稳定股骨粗隆间骨折的有效方法,锁定钢板是一种操作简单、创伤小、卧床时间短、并发症低、效果确切的治疗方法。
