超声检查中青年右侧锁骨下动脉内膜与硬化的关系研究

目的 分析超声检查中青年右侧锁骨下动脉内膜增厚性病变与动脉硬化之间的关系。方法 选择2019年7月至2021年6月浙江省桐乡市第四人民医院行颈部血管超声检查中的550例右侧锁骨下动脉超声检查的中青年患者,均确诊为冠状动脉粥样硬化性心脏病,并对其冠状动脉CT血管造影及颈部血管超声检查的影像学资料进行回顾性分析。应用超声下血管内中膜厚度对颈部血管病变进行评估,观察并分析中青年冠状动脉粥样硬化性心脏病病患颈动脉内膜增生的发生情况。结果 所选择的研究对象颈动脉超声下血管内膜厚度均出现增厚现象。右侧锁骨下动脉起始部血管内膜厚度出现增厚473例,且血管内膜厚度概率高于右锁骨下动脉起始部位发生率。结论 对于中青年冠状动脉粥样硬化性心脏病患者而言,发生右侧锁骨下动脉内膜增厚的概率较颈动脉球部的概率低,因此对此类患者可行超声右侧锁骨下动脉血管内膜厚度检查,从而帮助确诊以及评判病情严重程度。

Study on the relationship between the right subclavian artery intima and atherosclerosis in young and middle-aged people by ultrasonography

Objective To study the relationship between the intimal thickening lesions of the right subclavian artery and arteriosclerosis in young adults. Methods A total of 550 young and middle-aged patients who underwent ultrasound examination of right subclavian artery in the Fourth People''s Hospital of Tongxiang hospital from July 2019 to June 2021 were diagnosed as coronary atherosclerotic heart disease. The imaging data of coronary CT angiography and carotid vascular ultrasound were retrospectively analyzed. The intima-media thickness was used to evaluate the carotid vascular lesion under ultrasound. The occurrence of carotid intima hyperplasia and intima-media thickness at the beginning of right subclavian artery in young and middle-aged patients with coronary atherosclerotic heart disease were observed and analyzed. Results The intima thickness of carotid artery was increased by ultrasound in 550 cases of right carotid bulb, 512 cases of bilateral carotid artery and 512 cases of left carotid artery. The intima thickness of the right subclavian artery was increased in 473 cases, and the incidence of intima thickness was higher than that of the right subclavian artery. Conclusion For young and middle-aged patients with coronary atherosclerotic heart disease, the incidence of intimal thickening of the right subclavian artery is lower than that of the carotid bulb. Therefore, it is feasible for these patients to have an ultrasonal right subclavian artery intima thickness test to help diagnose and judge the severity of the disease.
