高压氧联合真空封闭引流技术在前臂带蒂旋转皮瓣修复手背创伤的应用

目的探讨高压氧联合真空封闭引流技术在前臂带蒂旋转皮瓣修复手背创伤的应用.方法选取80例手背创伤患者为研究对象,按照随机数表法分为研究组与对照组.对照组采用常规治疗联合真空封闭引流技术,研究组在对照组基础上联合高压氧治疗.比较治疗24d时,两组患者治疗效果评估差异;比较两组患者创面愈合时间、创面皮肤血运恢复时间、住院时间评估差异;比较治疗前及治疗24d时,两组患者血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)、S100A9水平变化.结果治疗24d时,研究组患者治疗效果优良率明显高于对照组(P<0.05);研究组患者创面愈合时间、创面皮肤血运恢复时间、住院时间明显少于对照组(P<0.05);治疗24d时,两组患者VEGF、bFGF、S100A9水平均较治疗前显著升高(P<0.05),且研究组明显高于同一时间对照组(P<0.05).结论高压氧联合真空封闭引流技术应用于前臂带蒂旋转皮瓣修复手背创伤患者,可提高治疗效果,促进创面及血清指标恢复,缩短患者住院时间.

Application of hyperbaric oxygen combined with vacuum sealing drainage technique in the repair of hand back trauma with forearm pedicled rotating flap

Objective To explore the application of hyperbaric oxygen combined with vacuum sealing drainage technique in the repair of hand back trauma with forearm pedicled rotating flap.Methods Eighty patients with hand back trauma were selected as research subjects,and they were divided into study group and control group according to the random number table method.Control group was given conventional therapy combined with vacuum sealing drainage technique,and study group was combined with hyperbaric oxygen therapy on the basis of control group.At 24 d after treatment,the treatment effects were compared between the two groups.The wound healing time,wound skin blood supply recovery time and hospital stay were compared between the two groups.The levels of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)and S100A9 were compared between the two groups before treatment and after 24 d of treatment.Results At 1d after treatment,the excellent and good rate of treatment effects in study group was significantly higher than that in control group(P<0.05).The wound healing time,wound skin blood supply recovery time and hospital stay in study group were significantly shorter than those in control group(P<0.05).At 24 d after treatment,the levels of VEGF,bFGF and S100A9 in the two groups were significantly increased compared with those before treatment(P<0.05),and the levels in study group were significantly higher than those in control group at the same time(P<0.05).Conclusion Hyperbaric oxygen combined with vacuum sealing drainage technique in the repair of hand back trauma with forearm pedicled rotating flap can improve the treatment effects,promote the recovery of wounds and serum indicators,and shorten the hospital stay.
