布托啡诺复合丙泊酚在无痛支气管镜检查中的麻醉价值

目的布托啡诺与芬太尼分别与丙泊酚复合，用于无痛支气管镜检查的麻醉效果。方法将60例ASAI～II级行无痛支气管镜检查的患者随机分为两组，布托啡诺组30例（A组），芬太尼组30例（B组），行静脉全麻，采用多功能监测仪监测患者麻醉前（T0）、注药后2min（T1）、支气管镜置入（T2）及退出（T3）时等不同时间点的平均动脉压（MAP）、心率（HR）、指氧饱和度（SpO2），并记录麻醉起效时间，苏醒时间，离院时间，麻醉后不良反应及并发症。结果两组麻醉效果满意度无显著性差异，但血流动力学、呼吸抑制两组间差异有显著性（P〈0．05），苏醒时间及离院时间差异有显著性（P〈0．05），处于可接受范围之内。结论布托啡诺复合丙泊酚用于无痛支气管镜检查对患者循环和呼吸功能抑制程度较轻，不良反应少，并发症发生率低，是一种理想的麻醉方法。

Clinical Value of Butorphanol Combined with Propofol for Patients Undergoing Painless Bronchoscopy

Objective To explore the clinical efficacies of butorphanol and fentanyl when both of them combined with propofol for the patients undergoing painless bronchoscopy.Methods Sixty patients of Anesthesiologists grades I and II,who asked for painless bron-choscopy,were recruited in this study.The patients were randomly assigned to group A(butorphanol plus propofol,n=30)and group B(fentanyl plus propofol,n=30).The mean artery pressure(MAP),heart rate(HR),and saturation pulse oxygen(SpO2) were recorded at different time points(before medication(T0),2 min after medication(T1),bronchoscopy starting(T2) and bronchoscopy ending(T3).We also recorded the time of anesthesiat,he interval of stopping administration to eye opening,and the time of hospital discharge,and an-esthetic effectt;he intra-and postoperative side effects were also observed.Results Anesthetic effect of satisfaction showed no signifi-cant difference between two groups.But haemodynamic and respiratory depression were significantly different between the two groups(P<0.05)r,ecovery time and discharged time were significantly different between the two groups(P<0.05),which can be guaranteed.Conclusion Butorphanol combined with propofol for patients undergoing painless bronchoscopy is safe and butorphanol has less inhibi-tory respirationf,ewer side effects and lower complication.It is ideal for painless bronchoscopy.
