太原市某社区Ⅱ型糖尿病人基本健康状况及肾功能影响因素的分析

目的 了解太原市某社区Ⅱ型糖尿病患者的基本健康状况,并初步探讨糖尿病患者肾功能损伤情况及其影响因素.方法 以太原市某社区270例(男性109例,女性161例)Ⅱ型糖尿病患者为研究对象,通过凋查表收集研究对象一般情况,并对其进行相关体格检查,采集相应的生物样品进行实验室检查.结果 该社区糖尿病患者中男性腰臀比( waist-hip ratio,WHR)高于女性,差异有统计学意义(P＜0.05),年龄在70～岁组患者WHR高于其他年龄组；80岁以上者尿N-乙酰-β-D氨基葡萄糖苷酶(N-acetyl-beta-glucosaminidase,UNAG)水平低于其他年龄组.以血糖水平7.0 mmol/L作为截断值将人群分为两部分后进行分析,发现血糖≥7.0 mmol/L者WHR和UN-AG的排出量均高于血糖＜7.0 mmol/L者,差异均有统计学意义(均有P＜0.05).多因素Logistic回归分析结果显示,病程和WHR为该社区糖尿病患者血糖水平≥7.0 mmol/L的危险因素.WHR、吸烟和UNAG为尿白蛋白(urinary albumin,UALB)增高的危险因素,体质指数、空腹血糖和UALB为UNAG增高的危险因素.结论 控制体重,降低WHR、控制血糖可以减缓糖尿病的进程.糖尿病患者空腹血糖水平升高是肾功能损伤的危险因素.

Epidemiological survey on health status and influencing factors of renal function among type 2 diabetes mellitus in a community of Taiyuan City

Objective To investigate the health status of type 2 diabetic patients,explore the renal dysfunction and the influencing factors of these patients.Methods 270（109 males,161 females） type 2 diabetes mellitus patients from a community in Taiyuan City were selected as subjects.General information were collected by questionnaire,and physical examine were used to collect data.Results The waist-hip ratio（WHR） of males was higher than females,and subjects that age range from 70 to 79 showed higher WHR,age≥80 showed lower N-acetyl-beta-glucosaminidase（UNAG）.Subjects with fasting plasma glucose（FPG） more than 7.0 mmol/L showed higher UNAG and WHR（all P〈0.05）.The result of multivariate Logistic regression showed that the course of disease and WHR were risk factors of FPG.WHR,smoking and UNAG were risk factors of urinary albumin（UALB）.And BMI,FPG and UALB showed the relations with UNAG.Conclusions For type 2 diabetic patients,controlling weight,keeping WHR,decreasing FPG should be regarded as primary preventive measures.
