介入性隔离动脉内灌注治疗下肢骨肉瘤

目的：探讨介入性隔离动脉内灌注（Interventional Isolated Limb Perfusion IILP)治疗下肢骨肉瘤的疗效。方法：采用介入技术隔离下肢血液，动脉内灌注化疗骨肉瘤17例，IILP后视病情二期保肢手术，最长随访75个月。结果：疼痛消失，肿瘤缩小，关节功能改善，肿瘤治疗有效率94．1％，16例二期切除肿瘤达保肢目的：局部复发率11．7％，手术切除肿块坏死率90％以上占58．8％，造影与病理相符，结论：介入性隔离动脉内灌注化疗安全，有效，对肿瘤细胞具很大的杀伤作用，是保肢治疗的有效方法。

Treatment of Osteosarcoma of the Lower Extremities with Interventional Isolated Limb Perfusion

Objective To investigate the efficacy of osteosarcoma of the lower extremities treated by interventional isolated limb perfusion (IILP).Methods 17 patients with lower limb osteosarcoma accepted interventional isolated limb perfusion, then surgical operation were done according to their actual situation. They were followed up routinely (the longest time is 75 months).Results Pain disappearing, tumor mass shrinking and joint function improving, the effective rate was 94.1%. The tumor secondary resections were done in 16 patients with reserving the lower limbs. The local recurrence rate was 11.7%. Tumor necrosis was more than 90%. Angiographic and pathological finding were coincident.Conclusions Interventional isolated limb perfusion would be an effective and minimal invasive method for treatment of osteosarcoma of the lower extremities.
