恶性肿瘤患者外周血T细胞亚群的检测及其临床意义

目的检测分析恶性肿瘤患者外周血CD3 ,CD3 CD4 ,CD3 CD8 比例及CD3 CD4 /CD3 CD8 比值的变化,并探讨其临床意义。方法采用多参数流式细胞术(FCM)检测254例恶性肿瘤患者外周血T细胞亚群水平与31例健康者比较研究。结果与正常人比较,恶性肿瘤患者外周血CD3 ,CD3 CD4 细胞比例明显减少(P<0.01),而CD3 CD8 细胞比例明显增多(P<0.01),CD3 CD4 /CD3 CD8 比值明显下降(P<0.01)。性别、各年龄阶段不影响T细胞亚群水平的表达,差异无统计学意义(P>0.05)。鼻咽癌患者CD3 CD4 细胞比例及CD3 CD4 /CD3 CD8 比值明显低于其他种类肿瘤患者(P<0.01)。结论恶性肿瘤患者细胞免疫功能紊乱,监测肿瘤患者T细胞亚群水平,对评估患者的细胞免疫功能及疾病的诊断和预后判断具有积极的临床意义。

Alterations and significance of T lymphocyte subsets in peripheral blood in patients with carcinoma

Objective To detect the level of T lymphocyte subsets in the peripheral blood and evaluate its clinical significance in patients with carcinoma. Methods The total T lymphocytes (CD3 cells) and T lymphocyte subsets (CD3 CD4 cells and CD3 CD8 cells) were detected by flow cytometry in the peripheral blood of carcinoma patients (n=254) and the healthy persons (n=31) as control, and the ratio of CD3 CD4 /CD3 CD8 was calculated. Results Compared with the healthy persons, the patients with carcinoma showed a significant decrease in the percentage of CD3 cells, CD3 CD4 cells and the ratio of CD3 CD4 /CD3 CD8 , P<0.01; in contrast, the percentage of CD3 CD8 cells was significantly increased, P<0.01. The age and gender had no influence on the level of T lymphocyte subsets, P>0.05. Conclusions These changes indicate that there is decreased cellular immunity in patients with carcinoma and might provide important information for the diagnosis of various kinds of carcinomas in clinic.
