超声检测外周动脉预测冠状动脉病变的相关性研究

摘    要:目的探讨超声检测外周动脉粥样硬化情况和内皮功能对冠状动脉血管病变的临床预测价值。方法应用彩色多普勒超声对72例健康者及56例明确冠状动脉病变患者的颈动脉内-中膜厚度(carotid inti-ma-media thickness,CIMT)、股动脉内-中膜厚度(femoral intima-media thickness,FIMT)及肱动脉内皮功能进行检测,比较两组的检测结果并进行统计学分析。结果冠状动脉病变组患者的CIMT、FIMT值均高于健康组(P<0.05),其颈动脉、股动脉粥样斑块检出率明显升高,而肱动脉内皮依赖性舒张功能(flow-mediated dilation,FMD)明显低于健康对照组(P<0.05)。结论超声检查外周血管可以预测冠状动脉病变,该检测方法具有无创、精确、方便、快捷等优势,可以作为预测冠脉病变的首选方法。

Abstract:Objective To observe the predictive effects of carotid,femoral artery atherosclerosis and Brachial artery endothelial function in coronary heart disease(CHD).Methods Using Ultrasound inspection of 72 healthy cases and 56 atherosclerosis cases that carotid intima-media thickness(CIMT),flow-mediated dilation(FMD),femoral intima-media thickness(FIMT),results were obtained and analyzed.Results The CIMT and FIMT of the coronary artery disease group were higher than the healthy group(P<0.05).The risks of atherosclerotic plaque detected in carotid arteries and femoral artery of coronary artery disease group were higher than that of the healthy group(P<0.05),but the FMD of coronary artery disease group were significantly lower than the healthy group(P<0.01).Conclusions The proposed method to measure periphery blood vessel is accurate and convenient,and there is potential to apply this method to diagnose coronary disease in clinic.
