Solitaire AB支架辅助弹簧圈栓塞13例颅内宽颈动脉瘤的临床分析

摘    要:目的:探讨应用Solitaire AB支架辅助弹簧圈栓塞治疗颅内宽颈动脉瘤的效果。方法:选取本院2010年1月-2013年9月采用Solitaire AB支架辅助栓塞治疗颅内宽颈动脉瘤病例13例,对患者的临床资料进行回顾性分析。结果:13例患者手术后取得满意疗效,置入支架13枚,支架一次性到位12枚,通过调整后到位1枚,放置位置满意;即刻血管造影致密栓塞10例,大部分栓塞2例,不全栓塞1例;出院时按改良Rankin评分0分12例,1分1例,无死亡病例。全部病例经1个月~3年随访,随访期间患者无一例死亡,无再出血及栓塞等症状。结论:Solitaire AB支架辅助栓塞颅内宽颈动脉瘤是一种安全可行和有效的治疗手段,近期随访疗效确切。

Clinical Analysis of 13 Patients with Intracranial Wide-necked Aneurysms by Using Solitaire AB Stent- assisted Coil Embolization

Abstract:Objective: To explore the techniques and efficacy of Solitaire AB stent-assisted coiling in treatment of intracranial wild-necked aneurysms. Method: A retrospective analysis was conducted for 13 patients with intracranial wild-necked aneurysms enrolled in our hospital from Feb 2010 to Sep 2013 , they were treated with Solitaire AB stent- assisted coiling. Result: 13 cases obtained satisfactory clinical outcomes, 13 stents were deployed accurately, 12 stents were deployed by one time, and 1 was deployed after adjustment.Immediate angiography demonstrated complete occlusion in 10 cases, neck remnant in 2 cases, and incomplete occlusion in 1 case.According to modifiled Rankin Scale on discharge from the hospital, 12 belonged in grade 0, 1 in grade 1. All cases followed up from 1 month to 3 years, there were no deaths, no further bleeding or other symptoms of embolism. Conclusion: The endovascular treatment of intracranial wild-necked aneurysm with Solitaire AB stent-assisted coil embolization is safe and effective, with definite short-term effectiveness.
