摘    要:目的研究改良的强制使用运动疗法对脑损伤患者上肢运动功能恢复的影响。方法30例病程在1~6个月的脑损伤偏瘫患者随机分为两组,15例接受常规的康复治疗;15例接受改良的强制使用运动疗法,在治疗期间,不固定患者健侧上肢,接受行为再塑的技巧训练,密集训练患侧肢体活动,完成日常生活中的动作,连续15个工作日,4h/d。治疗前、治疗后用Fugl-Meyer Assessment(FMA),Barthel指数评价患者的上肢功能。结果两组患者治疗前后的FMA、Bathel指数评分值比常规治疗组治疗后的分值提高更明显(P<0.01),差异有统计学意义(P<0.05)。结论改良的强制使用运动疗法是改善脑损伤患者上肢功能的有效治疗方法。

Abstract:Objective To determine the efficacy of modified constraint-induced movement therapy(mCIMT)protocol for patients with brain injury. Methods 30 hemiplegic patients with brain injury were randomly divided into conventional rehabilitation group(15cases) and mCIMT group(15cases). The patients were trained(by shaping) the more affected extremity without restriction of movement of intact upper extremity for 4 hours per day on the 15 week days during mCIMT period. Upper-extremity motor ability was assessed with Fugl-Meyer Assessment(FMA)and Bathel Index (BI) pre-and posttherapy. Results Upper-extremity motor ability was improved after therapy in both group, and the assessment scores in treatment group was obviously superior to that in conventional group. Conclusions mCIMT is an efficacious method of improving function and use of the more affected arms of patients with brain injury.
