充血性心力衰竭低钠血症的形成及其对预后的影响

目的探讨充血性心力衰竭(CHF)患者合并低钠血症的影响因素及低钠血症与心力衰竭转归的关系。方法检测152例CHF患者的血钠浓度,回顾性分析CHF患者血清钠浓度与年龄、利尿剂的使用、心力衰竭程度和病死率的关系。结果152例患者中有44例(28.9%)患者存在低钠血症,年龄大、使用利尿剂的患者低钠血症的发生率增加,低钠血症与心力衰竭的严重程度有关,低钠血症组的病死率高于血钠正常组。结论CHF患者出现低钠血症有多方面的原因,血清钠可作为CHF患者病情判断和预后观察指标。

The Form and the Influence of Prognosis in Congestive Heart Failure Combin with Hyponatremia

Objective To investigate the factors leading to congestive heart failure（CHF）and the relation between hyponatremia and prognosis of heart failure. Methods Determined the strength of serum sodium of 152 CHF patients, reviewed and analyzed the relation among the serum sodium, ages, use diureticum, sevwrity of heart failure and the possibility of death. Results There were 44 patients out of 152（28.9%） having hyponatremia. Patient at an older age, using diureticum had a higher possibility of being diagnosed with hyponatremia, hyponatremia had a very close relation with the severity of heart failure, The possibility of death in group with hyponatremia was higher than the normal group, The comparison and difference between these two group had a great significance in statistics（P〈0.01）. Conclusions Hyponatremia can be treated as an index for diagnosis and prognosis observation of CHF patients.
