无创正压通气救治慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的临床研究

目的 探讨无创正压通气（NIPPV）治疗慢性阻塞性肺疾病（COPD）合并Ⅱ型呼吸衰竭的治疗效果。方法 观察23例NIPPV治疗慢性阻塞性肺疾病（COPD）急性加重期并发Ⅱ型呼吸衰竭患者的血气指标变化（pH值、PaO2、PaC02），住院病程，气管插管率和病死率，并同以往未使用NIPPV治疗的25例COPD相似病例进行比较分析。结果 23例NIPPV治疗前后24h血气指标明显改善，住院病程缩短，气管插管率（P＜0．05～0．01）降低，同非NIPPV治疗的对照组比较差异有统计学意义。结论 NIPPV是治疗慢性阻塞性肺疾病急性加重期并发Ⅱ型呼吸衰竭的一种有效方法。

Clinical Study of Noninvasive Positive Pressure Ventilation for Treating COPD Patients with Type Ⅱ Respiratory Failure

Objective To evaluate the effect of noninvasive positive pressure ventilation （NIPPV） on COPD patients with type II respiratory failure. Methods Arterial blood gas index （ pH, PaO2, PaCO2 ）, course of disease in hospital, rate of endotracheal intubation and mortality of NIPPV group（23 COPD patients with type II respiratory failure, treated with NIPPV） were observed, and these index was compared with control group （ COPD patients without NIPPV treatment）. Results Arterial blood gas index were improved significantly in NIPPV group after treatment. The course of disease in hospital was significantly shorter, and the rate of endotracheal intubation （ P〈 0.05 ～0.01 ） was significantly lower in NIPPV group than those in control group. Conclusions NIPPV is an effective method in the treatment of acute exacerbations of COPD with type II respiratory failure.
