浆细胞性乳腺炎的MRI征象分析

目的：分析浆细胞性乳腺炎的MRI表现特点,进一步提高其影像诊断水平,探讨MRI的临床应用价值。方法：回顾性分析2007年11月-2009年4月间经临床手术病理证实的53例浆细胞性乳腺炎患者的MRI,分析病变部位、形态、周围腺体结构、乳后间隙及病变强化后的时间-信号强度曲线。结果：浆细胞性乳腺炎主要发生在乳晕后方及周围,本组有34例（34/53）,外上象限3例（3/53）,内上象限7例（7/53）,弥漫分布于乳腺内9例（9/53）。病灶于T1WI呈低或等信号,T2WI呈高或等高信号,增强后病灶全部明确显示。27例病变形成脓肿（27/53）,3例（3/53）病变呈斑片状信号异常,5例（5/53）表现为肿块样信号异常,18例（18/53）窦道或瘘管形成。结论：MRI能准确地发现并定位病灶,对临床诊断与治疗具有很大的意义。

MRI features of plasma cell mastitis

Objective：To analyze the image features of plasma cell mastitis on MRI and to improve the understanding and diagnostic performance of it.Methods：MRI data of fifty-three cases of pathologically confirmed plasma cell mastitis were reviewed retrospectively.The location,shape,surroundings and dynamic contrast time-intensity curve were analyzed.Results：The plasma cell mastitis were located behind or around mammilla（34/53）,at upper-lateral quadrant（3/53）,upper-medial quadrant（7/53）,or diffuse（7/53）.The lesions showed iso-or long T1 and T2 signal intensity and enhanced on contrast study.27 cases had abscesses,3 cases showed focal area enhancement,5 cases showed mass enhancement and 18 cases had sinus or fistula formation.Conclusion：MRI can detect and locate the plasma cell mastitis and it is helpful for its diagnosis and treatment.
