肿瘤标记物联合检测在肺癌诊断中的临床应用

目的 评价癌胚抗原(CEA)、血清细胞角蛋白19片段(CY211)、糖类抗原125(CA125)及神经元特异烯醇化酶(NSE)水平对肺癌早期诊断、判断病理类型中的意义.方法 检测89例肺癌患者和82例肺良性疾病患者血清CEA、CY211、CA125及NSE水平并进行比较分析.结果 肺癌患者血清CEA、CY211、CA125及NSE水平均高于肺良性疾病组,前三者差异有统计学意义(P＜0.05);其中CY211的敏感性优于其余3项指标,4项联合检测的敏感性优于单个指标,差异有统计学意义(P＜0.01),NSE在小细胞癌中敏感性最高,CEA在腺癌中敏感性最高,CY211在鳞癌中最高.结论 血清CEA、CY211、NSE及CA125均可作为肺癌筛查的指标;其中CY211最敏感.另外,不同的肿瘤标记物升高对不同的病理分型有一定帮助;4项联合检测可提高肺癌诊断的灵敏度、准确率.

Clinical Application of Tumor Markers in the Diagnosis of Lung Cancer

Objective To investigate the significance of tumor markers including eytoker atin 19 fragment(CY211),neuron-specific enolase(NSE),carcinoembryonic antigen(CEA)and Cancer antigen-125(CA125) in diagnosis and the histological differentiation of lung cancer.Methods Serum levels of CY211,NSE,CEA and CAl25 were detected in 89 patients with lung cancer and 82 patients with benign pulmonary disease.Those data were compared to analyze the diagnostic values of these tumor markers.Results A1l serum levels of tumor markers in the patients with lung cancer were higher than those in benign pulmonary disease group,the first three were significance difference(P<0.05).CY211 had the higher sensitivity than NSE,CEA,CAl25.The sensitivity of combined determination four tumor markers were hinger than single index.NSE had the highest sensitivity in SCLC,and CEA in adenocarcinoma,CY211 in squamous carcinoma.Conclusion CY211,NSE,CEA and CAl25 are all useful in lung cancer while CY211 has the highest sensitivity.Increased different tumor maekers are helpful to distinguish the histological differentiation of lung cancer.A combination of multiple tumor markers increase diagnostic value of the disease in terms of its sensitivity and accuracy.
