血清脂联素水平及踝臂指数与冠状动脉病变程度的相关性研究

摘    要:目的 观察冠心病(CHD)患者血清脂联素(APN)水平,踝臂指数(ABI),探讨其与冠状动脉病变程度之间的关系.方法 将86例拟诊CHD并接受冠状动脉造影的患者分为急性冠状动脉综合征(ACS)组29例、稳定型心绞痛(SAP)组36例、对照组21例,采用酶联免疫吸附法(ELISA)测定APN水平,测量所有患者的ABI.结果 ACS组、SAP组的APN水平均低于正常对照组(P<0.05),ACS组的APN水平低于SAP组(P<0.05),2支、多支病变组ABI水平明显低于1支病变组和对照组,差异有统计学意义(P<0.05).结论 APN是CHD的一个保护性因子,APN可以增加斑块的稳定性,减少ACS的发生;ABI与冠状动脉病变程度明显相关,APN与ABI联合检测在评估CHD患者冠状动脉病变程度方面有重要意义.

Abstract:Objective To explore the relationship between plasma adiponectin(APN) concentration and the ankle brachial index(ABI),the severity of coronary artery lesion in coronary heart disease(CHD).Methods The subjects were divided into ACS group including 29 patients,SAP group including 36 patients and control group including 21 patients.The plasma levels of APN were determined by ELISA and ABI were tested by the non-invasive vascular screening device.Results APN was significantly lower in ACS group and SAP group than control group(P<0.05),APN was significantly lower in ACS group compared to SAP group(P<0.05).ABI was significantly lower in patients with two and more vascular lesions than those in the control group and in one vascular lesion(P<0.05).Conclusion APN is a protective factor in coronary heart disease.APN may be able to promote stability of atherosclerotic plaques,reduce the incidence of ACS.There is significant diference between ABI and the severity of coronary artery stenosis.APN combine with ABI could provide a important way for evaluating and diagnosing the severity of coronary artery disease.
