川芎嗪、参麦注射液合用对肺心病肺动脉高压的影响研究

目的观察川芎嗪、参麦注射液合用对肺心病肺动脉高压的影响。方法 100例慢性肺心病急性期美国纽约心脏协会(NYHA)标准分级心功能Ⅲ～Ⅳ级患者按数字表法随机分为两组。对照组采用西医常规治疗,治疗组在西医常规治疗的基础上加用中成药川芎嗪注射液、参麦注射液进行治疗,观察两组治疗前后临床症状体征积分、肺动脉收缩压(PASP)、左室射血分数(LVEF)、体循环血压、6min步行试验(6-MWT)变化,并观察两组药物不良反应等。结果对照组临床显效率40%、总有效率84%,肺动脉高压(PAH)下降率17.83%,LVEF增加8.08%,6-MWT增加1.4级;治疗组临床显效率80%、总有效率98%,PAH下降率27.04%,LVEF增加10.36%,6-MWT增加2.1级。组间相应各项比较差异均有统计学意义(P<0.05)。结论在西医常规治疗的基础上联合川芎嗪、参麦注射液,可以有效降低PAH,逆转肺血管的病理变化,不会造成低血压、心搏出量下降等副作用,是治疗慢性肺心病急性期的一种有效、经济的治疗方案。

Effect of Ligustrazine and Shenmai Injection on Pulmonary Artery Hypertension in Cor Pulmonale

Objective To observe the effect of ligustrazine and shenmai injection on pulmonary artery hypertension(PAH) in cor pulmonale.Methods 100 patients of chronic cor pulmonale in acute stage with cardiac function class Ⅲ~Ⅳ on NYHA were randomly divided into control group and treatment group.The two groups were received routine therapy,but the treatment group was treated by ligustrazine and shenmai injection in addition.To observe symptoms,signs,sPAP,LVEF,systemic BP,6-MWT and adverse drug reactions in the two groups.Results The obvious effective rate,total effective rate,PAH depressed rate,LVEF increased rate and 6-MWT increased level of control group was respectively 40%,84%,17.83%,8.08% and 1.4.The treatment group was respectively 80%,98%,27.04%,10.36% and 2.1.There was statistical significance on items between the two groups(P<0.05).Conclusion Ligustrazine and shenmai injection may reduce pulmonary hypertension and reverse pathological change on pulmonary vascular.It couldn′t reduce blood pressure and CO.The result shows that it is a operative and economical method for patients on chronic cor pulmonale in acute stage.
