蔗糖铁注射液联合左卡尼汀治疗维持性血液透析患者肾性贫血的疗效观察

目的：观察比较蔗糖铁注射液联合左卡尼汀与口服铁剂两种方法治疗维持性血液透析肾性贫血的疗效与安全性。方法：将68例维持性血液透析肾性贫血患者随机分为观察组和对照组，各34例，观察组采用蔗糖铁注射液联合左卡尼汀治疗，每周2次，均于每周第1、2次透析后3h使用，对照组口服右旋糖酐铁300mg／d。治疗时间均为5N，于第8周时观察两组疗效。结果：观察组治疗后Hb、HCT、SF、TSAT均较治疗前有显著提高（P〈0．05），EPO使用量较治疗前明显减少（P〈0．05），治疗后显效20例，有效9例，总有效率为85．29％；对照组治疗后Hb、HCT较治疗前均有所提高，但变化不大，SF、TSAT较治疗前有明显提高，EPO使用量无明显变化，治疗后显效13例，有效10例，总有效率为67．65％，两组在显效例数和总有效率上比较，差异有统计学意义（P〈0．01）。结论：蔗糖铁注射液联合左卡尼汀治疗维持性血液透析肾性贫血具有起效快，易准确估算补铁剂量，不良反应较口服铁剂轻微，患者依从性更好，能有效改善患者的贫血症状，是一种安全有效，值得推广的临床治疗方法。

Results： The observation group after treatment, HCT, SF Hb TSAT were relatively before treatment, significantly improve （P〈0.05）, EPO usage were reduced significantly than that before treatment （P〈0.05）, after treatment, marked effect 20 cases, effective 9 cases, with the total effective rate was 85.29%; The control group after treatment than before treatment, HCT, Hb were improved, but had not changed much, SF, TSAT were improved obviously than that before treatment, EPO usage without opparent change, after treatment, marked effect 13 cases, effective 10 cases, the total effective rate was 67.65%, two groups in powerfully number and the total effective rate was significant difference （P〈 0.01）. Conclusion： Cane sugar iron injection joint levo eamitine treatment maintenance hemodialysis renal anemia effectacting fast, easy with iron dose for estimate accurately, and adverse reaction is mild, patients oral iron agent compliance better, can effectively improve symptoms of anemia patients, it is a safe and effective, and worthy of popularizing in clinical treatment.
