微创技术在处理并发继发病变的输尿管上段结石中的应用研究

目的:探讨两种微创技术在处理并发继发病变的输尿管上段结石中的疗效、安全性,提出选择治疗术式的依据。方法:回顾分析经尿道输尿管镜取石术(URSL)及经皮肾微造口输尿管肾镜取石术(PCNL)治疗输尿管上段结石388例的临床资料。结果:经尿道组286例,治疗成功率92.3%(264/286),一次结石取净率83.3%(238/286),26例加用ESWL治疗,成功率100%(26/26),失败率7.69%(22/286),并发症发生率5.6%(14/286)。经皮肾组102例,治疗成功率98.04%(100/102),2例加用URSL,成功率100%(2/2),一次结石取净率100%,无手术相关并发症发生。结论:对并发继发病变的输尿管上段结石,选用PCNL治疗安全性、有效性更好。

The realization in treating upper ureteral calculi of merger pathological changes of different minimally invasive technology

Objective:To study the effect and safety in treating upper ureteral calculi of merger pathological changes of different minimally invasive technology.Methods:Retrospective study of 388 cases of complicated upper urinary calculi treated with URSL and PCNL.Results:The success rate of URSL was 92.3%(264/286).The stone free rate was 83.3%(238/286) at 1 month follow-up. 26 patients with residual required ESWL combination. The failure rate was 7.69%(22/286) ,The complication rate was 5.6%(14/286). The success rate of PCNL was 98.04%(100/102).2 patients with residual required URSL,the stone free rate was 100% and no complication was noted.Conclusions:Success rate and stone free rate of PCNL were better than URSL for patients with upper ureteral calculi of merger pathological changes.
