舒适护理对初产妇妊娠结局和泌乳功能的影响

目的：观察舒适护理对初产妇妊娠结局及泌乳功能的影响。方法：将120例初产妇随机分为两组,每组60例,常规组实施常规护理,舒适组实施舒适护理,观察妊娠结局,采用焦虑自评表（SAS）、抑郁自评表（SDS）评价产妇产前产后情绪,采用视觉模拟评分法（VAS）评估产妇疼痛情况,测量产后泌乳量。结果：两组胎儿窘迫发生率、新生儿窒息率比较差异无统计学意义（P〉0.05）;舒适组剖宫产率、产后出血率、围产期VAS评分、产后SAS和SDS评分低于常规组（P〈0.05）;舒适组泌乳始动时间短于常规组（P〈0.01）;舒适组产后24h、48h泌乳量均多于常规组（P〈0.01）。结论：舒适护理可消除初产妇不良情绪,改善妊娠结局,并提高泌乳量。

Impact of comfort care on pregnancy outcomes and lactation of primipara

Objective To observe the effects of comfort care on pregnancy outcomes and maternal lactation functions of primipara.Methods A hundred and twenty parturients were equally randomized into conventional group（n=60）and comfort care group（n=60）,and received routine care and comfort care respectively.Then we observed the pregnancy outcomes and measured the amount of postpartum lactation.Meanwhile,we used self-rating anxiety scale（SAS）and self-rating depression scale（SDS）to assess the pre-and post-partum emotions of primipara,used visual analogue scale（VAS）to evaluate the maternal pain.ResultsCompared with the conventional group,there were no significant differences in the incidence of fetal distress and the rate of neonatal asphyxia（P〉0.5）.The rates of cesarean section,postpartum hemorrhage,the perinatal VAS score,postpartum SAS and SDS scores of the comfort group were lower than those in the conventional group（P〈0.05）.And the initial time of lactation of the comfort group was shorter than that of the conventional group（P〈0.01）.The postpartum 24 h,48h milk yield of comfort group was also more than that of the conventional group（P〈0.01）.Conclusions Comfort care can eliminate the negative emotions of primipara,improve pregnancy outcomes and increase milk yield.
