41例医院深部真菌感染的临床分析

目的 了解医院深部真菌感染的临床特点，为减少医院真菌感染提供依据。方法 对2001年1月-2002年5月出院患者真菌培养阳性病例进行回顾性分析。结果 41例医院深部真菌感染患者平均年龄53．4岁，均有基础疾病，多发于老年人，占感染病例65．9％；感染部位以下呼吸道为最多；真菌感染以白色念株菌居多，占55．6％；所有患者均使用过抗菌药物，主要是第三代头孢菌素和睦诺酮类，累计使用种类多、时间长且多为联合用药；抗真菌治疗不规范。结论 医院深部真菌感染主要危险因素是不合理使用抗菌药物，临床医生应重视早期病原学检查，根据细菌培养及药敏试验结果合理选用抗菌药物；规范抗真菌治疗方法，同时注意加强营养和支持疗法。

Nosocomial Systemic Fungus Infection: A Clinical Analysis of 41 Cases

OBJECTIVE To investigate the clinical features of nosocomial systemic fungus infection. METHODS The fungus cultured positive cases among the discharged patients from Jan 2001 to May 2002, were reviewed and analyzed. RESULTS The average age of 41 cases with nosocomial systemic fungus infection was 53.4 years old, mainly senile patients with various diseases accounted for 65.9%. Lower respiratory tract infection was the most frequent infection site. The Candida albicans was accounted for 55.6%. Antibiotics were used to all the patients, mainly third generation cephalosporins and quinolones, which were used alone, but mainly in combination for a long period of time. Anti fungus treatments were not standardized. CONCLUSIONS The main dangerous factors of the infection are the irrational use of antibiotics. Doctors should lay emphasis on the early examination of etiology and choose drugs properly according to cultivation and drug susceptibility tests, and standardize the ways of treatment, and pay more attention to nutrition and supporting therapy.
