眶内球后肿瘤的外科治疗探讨

目的探讨眶内球后肿瘤的外科治疗方法和疗效。方法对2000年至2006年收治的28例眶内球后肿瘤患者采用额下眶顶入路或眶外侧入路手术治疗,并根据病理诊断指导术后治疗。结果28例患者中,肿瘤全切除24例,次全切除3例,部分切除1例。术后视力改善4例（14.3%）,无变化22例（78.6%）,恶化2例（7.1%）。14例恶性肿瘤术后行放疗和/或化疗。随访6～14个月,肿瘤均无复发。结论手术是治疗眶内球后肿瘤的主要方法。手术虽不能明显改善术后视力,但对维系术前视力有帮助。恶性肿瘤术后还应行放疗和/或化疗。

Surgical Management of Retrobular Tumors

Objectives To explore the surgical treatment of retrobular tumors and its curative effect. Methods The clinical data of 28 patients with retrobular tumors treated in our department from 2000 to 2006 were analyzed retrospectively. All the patients underwent surgery through the subfrontal unroofing or Kr？nlein＇s approach. The postoperative radiotherapy or/and chemotherapy were performed in 14 patients with the malignant tumors. Results Total resection of the tumors was achieved in 24 cases,subtotal resection in 3 cases,the partial resection in 1 cases. The postoperative visual acuity was improved in 4 patients （14.3%）,unchanged in 22 patients （78.6%）,and worsened in 2 patients（7.1%）. The tumors did not recurred in all the patients during the follow-up period from 6 to 14 months. Conclusions Surgery is a main method to treat the retrobular tumors. The visual acuity may not be improved by the surgery in the patients with retrobular tumors,but the surgery is helpful to keeping preoperative visual acuity in the patients with retrobular tumors. Radiotherapy or/and chemotherapy should be performed in the patients with malignant retrobular tumors.
