CD40基因启动子区-1C/T多态性与早发急性心肌梗死的相关性研究

摘    要:目的:研究我国湖北十堰地区汉族人群CD40基因启动子区-1C/T多态性与早发急性心肌梗死(AMI)的相关性.方法:应用流式细胞仪俭测161例早发AMI患者和186名健康对照者的B淋巴细胞CD40表达水平;用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)检测CD40基因启动子区-1C/T多态性的基因型频率分布;应用酶联免疫吸附试验检测血清可溶性CD40配体(sCD40L)水平;应用微粒子增强透射免疫分析技术检测血清超敏C-反应蛋白(hs-CRP)水平?结果:早发AMI患者B淋巴细胞CD40表达水平高于对照组,且差异有统计学意义(平均荧光强度为6.81±2.16比2.08±1.19.P〈0.01):而其血清sCD40L及hs-CRP也均较对照组升高,差异有统计学意义(P均〈0.01)。基因型分布在早发AMI组(CC=34.2%、CT=54.7%、TT=11.1%)与对照组(CC=23.7%、CT=56.9%、TT:19.4%)的差异有统计学意义(P〈0.05):早发AMI组C等位基因频率高于对照组,差异有统计学意义(61.5%比52.2%,OR=L465,95%CI:1.082-1.983,P〈0.05);B淋巴细胞CD40表达水平在CC、CT、TT各基因型间差异也有统计学意义(P〈0.05)。结论:CD40基因启动子区-1C/T多态性可能与我国湖北十堰地区汉族人群早发AMI的发生有关。

Abstract:Objective To investigate the relationship between CD40 gene promoter region -1C/T polymorphism and early onset acute myocardial infarction (AMI) in Hans from Shiyan, Hubei Province of China. Methods One hundred and sixty-one definite early onset AMI patients (AMI group) and 186 healthy controls (control group) were recruited. The expression level of CD40 in B lymphoeytes was determined by flow cytometry and polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was used to determine the genotype frequency distribution of CD40-1C/T polymorphism. Serum soluble CD40 ligand (sCD40L) was measured by ELISA and serum hypersensitive C-reactive protein (hs-CRP) was determined by particle enhanced-turbidimetric immunoassay. Results Expression level of CD40 was higher in the AMI group than that in the control group (mean fluorescence intensity 6.81±2.16 vs 2.08±1.19, P〈0.01); serum sCD40L and hs-CRP were obviously elevated in the AMI group. The difference of genotype distribution was statistically significant between AMI group (CC=34.2%, CT=54.7%, 3T=l 1.1%) and the control group (CC=23.7%, CT=56.9%, TF= 19.4%). The fiequency of C allele was higher in the AMI group than that in the control group (61.5% vs 52.2%, OR= 1.465,95%CI:1.082-1.983,P〈0.05). Conclusions CD40 gene promoter region-1C/T polymorphism might be correlated with the development of early onset AMI in Hans from Shiyan, Hubei Province of China.
