经尿道前列腺电切术治疗晚期前列腺癌合并膀胱出口梗阻的疗效观察

目的探讨经尿道前列腺电切术（TURP）治疗晚期前列腺癌合并膀胱出口梗阻（BOO）的疗效。方法对24例前列腺癌合并BOO症状的患者采用经尿道前列腺电切术同时切除双侧睾丸，术后使用抗雄激素药物治疗，比较治疗前、后患者的临床症状、国际前列腺症状评分（IPSS）、前列腺特异性抗原（PSA）水平及最大尿流率（Qmax）的变化。结果手术后患者排尿困难、尿潴留、血尿等症状显著改善；与手术前比较，手术后患者IPSS评分和血清PSA水平均显著下降，最大尿流率明显增加（均P〈0．05）。结论经尿道前列腺电切术是治疗晚期前列腺癌合并B00的有效方法。

Experience of Transurethral Resection of Prostate in Patients with Advanced Prostate Cancer with Bladder Outlet Obstruction

Objective To probe the experiences of transurethral resection of the prostate for the treatment of prostate cancer with bladder outlet obstruction. Methods 24 prostate cancer patients with BOO definitely diagnosed by biopsy were treated by transurethral resection of the prostate combined with castrating and medical endocrinal therapy. We reviewed the changes of clinical symptoms,IPSS,PSA and Qmax between pre-operation and post-operation. Results The clinical symptoms of 24 patients were improved remarkably. IPSS and PSA reduced and the maximum flow rate significantly increased after the operation（all P〈0.05）. Conclusion Transurethral resection of the prostate is an ideal method for the treatment of advanced prostate cancer with bladder outlet obstruction.
