腹壁下动脉穿支皮瓣带蒂移植修复股部残端软组织缺损

目的报告腹壁下动脉穿支皮瓣带蒂移植修复股部残端软组织缺损临床应用结果。方法自2006年1月~2012年1月,应用腹壁下动脉穿支皮瓣带蒂移植修复股部残端软组织缺损12例(男9,女3)。患者年龄22~55岁(平均36岁)。伤后时间:6~28天,平均12天。股部残端软组织缺损范围5cm×10cm^6cm×14cm。皮瓣以腹壁下动脉穿支为蒂,旋转180°修复残端缺损,供区直接缝合。结果 1例皮瓣发生小的皮缘裂开,术后1月自然愈合,皮瓣全部成活。术后随访1.8~4.0年(平均2.8年),受区外形较好。取得了满意的临床效果。结论腹壁下动脉穿支皮瓣带蒂移植适宜修复股部残端软组织缺损,该技术安全、可靠以及可降低对供区的损伤。

Soft-tissue defect coverage in the stump of the thigh with a pedicled deep inferior epigastric perforator flap

[Abstract] Objective To report clinical application results of the repair soft tissue defect in the stump of the thigh with a pedicled deep inferior epigastric perforator flap. Methods From January 2006 to January 2012, 12 patients(9 men, 3 women) with soft-tissue defect in the stump of the thigh underwent reconstruction with a pedicled deep inferior epigastric perforator flap transplantation. They ranged in age from 22 to 55 years(mean 36y). Time after injury was from 6 days to 28 days(mean 12 days). soft-tissue defect size in the stump of the thigh was from 5cm×10cm to 6cm×14cm. Flap was based on deep inferior epigastric perforator and rotated to 180 degrees for repair soft tissue defect in the stump. The donor site was closed directly. Results One patient developed small wound dehiscence, which spontaneous healed at one month after surgery. All the flaps had survived completely. Follow-up period ranged form 1.8~4.0 years(mean 2.8y) postoperatively. A good contour was confirmed at the recipient area. Satisfactory clinical results were obtained in all cases. Conclusion The pedicled deep inferior epigastric perforator flap for the stump defects of the thigh is a good option. This technique is safe and reliable, and with minimal donor site morbidity.
