手法整复与手术治疗踝关节骨折的效果比较

目的比较手法整复与手术治疗踝关节骨折的临床效果。方法随机选取2014年1月至2017年1月踝关节骨折患者100例,依据治疗方案分为两组,每组50例。手法组患者接受手法整复外固定治疗,手术组患者接受切开复位内固定手术治疗。比较两组患者预后指标、功能评分及治疗效果。结果手法组患者住院时间、骨折愈合时间、完全负重时间与手术组比较,差异未见统计学意义(P>0.05)。治疗3个月后,两组患者踝关节功能评分、踝-后足功能评分高于治疗前(P<0.05),两组功能评分比较差异未见统计学意义(P>0.05)。手法组并发症发生率为6%,手术组为10%;手法组治疗优良率为90%,手术组为92%,两组比较差异均未见统计学意义(P均>0.05)。结论手法整复与切开复位内固定手术治疗踝关节骨折,均可有效改善患者的踝足功能,且安全可靠。临床可根据患者个体情况合理选择治疗方案。

Comparsion of effects of manipulative reduction and operative treatment on ankle fracture

Objective To compare the effect of manipulation and surgical on ankle fracture.Methods A total of 100 patients with ankle fracture from January 2014 to January 2017 were randomly selected,and they were divided into two groups according to the treatment plan,with 50 cases in each group.The patients in the manipulation group were treated by external fixation,and the patients in the operation group were treated with incision reduction and internal fixation.After 3 months of treatment,the prognostic index,functional score and therapeutic effect in the two groups were compared.Results The hospitalization time,fracture healing time,complete weight bearing time were compared between the two groups,there was no significant difference(P>0.05).After 3 months of treatment,the ankle function score and ankle-back foot function score of the two groups were higher than that before treatment(P<0.05),There was no significant difference in functional scores between the two groups(P>0.05).The incidence of complications in the manipulation group was 6%,and that in the operation group was 10%;the good rate of the manipulation group was 90%,and that of the operation group was 92%,and the difference between the two groups was not significant(P all>0.05).Conclusion Manipulation and open reduction internal fixation surgery for ankle fracture,both can effectively improve the patient's ankle foot function,and is safe and reliable.Clinically,the treatment plan can be selected reasonably according to the individual situation of the patient.
