摘    要:目的分析心肺运动功能测试(CPET)评估制定运动处方强度指导对慢性心力衰竭患者血压、CPET指标以及生命质量的影响。方法选取2018年1月至2018年3月慢性心力衰竭(CHF)患者50例,入院后均给予常规治疗及护理,并进行CPET评估,根据评估结果制定踏车运动康复处方,根据运动负荷强度分为对照组(n=25)和研究组(n=25),分别以80%无氧阈水平和无氧阈以上50%功率强度进行12周心脏运动康复干预。比较两组患者的血压、心率、CPET指标以及生命质量、超声心动图指标。结果干预前后,两组患者心率、舒张压以及收缩压比较,差异未见统计学意义(P>0.05);干预后12周,研究组明尼苏达心力衰竭生活质量调查表(MLHFQ)评分显著低于对照组,6 min步行试验(6MWD)显著高于对照组,差异有统计学意义(P<0.05);研究组峰值氧脉搏、峰值摄氧量以及无氧阈均显著高于对照组,差异有统计学意义(P<0.05);两组患者均未出现不良事件。结论在CPET评估制定运动处方强度指导下,高强度的心脏康复运动可以显著提高CHF患者的生命质量、心肺功能以及运动耐力,且不增加不良事件的发生,值得临床重视。

Abstract:Objective To analyze the effects of exercise intensity from cardiopulmonary exercise testing(CPET) on blood pressure,CPET index and quality of life in patients with chronic heart failure.Methods Fifty cases of CHF in to our hospital from January 2018 to March 2018 were selected,all patients were given routine treatment and nursing,and received CPET assessment.Based on the assessment results,the rehabilitation exercise for bicycle exercise was formulated,according to the intensity of exercise load,the patients were divided into control group(n=25) and study group(n=25).The patients exercised using 80% anaerobic threshold and exercised using exercise intensity of 50%power above anaerobic threshold,respectively.And underwent cardiac exercise rehabilitation intervention for 12 weeks.The blood pressure,heart rate,CPET index,quality of life and echocardiography were compared between the two groups.Results Before and after intervention,there were no significant difference in heart rate,diastolic blood pressure or systolic blood pressure between the two groups(P>0.05).After 12 weeks of intervention,the MLHFQ score of the study group was significantly lower than that of the control group,and 6 MWD was significantly higher than that of the control group,the difference were significant(P<0.05).The peak oxygen pulse,peak oxygen uptake and anaerobic threshold in the study group were significantly higher than those in the control group,the difference were significant(P<0.05);There were no adverse events in the two groups.Conclusions Under exercise intensity from cardiopulmonary exercise testing,high intensity cardiopulmonary exercise can significantly improve the quality of life,cardiorespiratory function and exercise tolerance of CHF patients,and no increase of adverse events,it is worth clinical attention.
