某警队1起甲型H1N1流感暴发调查

目的调查2009-10某警队1起甲型H1N1流感暴发的疫情规模、传染来源及传播途径。方法制定病例定义,开展病例主动搜索和病例对照研究。结果首发病例为另1起确诊甲流疫情的密切接触者,归队后发病,之后陆续57人发病,罹患率20%。病例集中在新警大队,在各中队、班和楼层的分布差异均无统计学意义,但存在宿舍聚集性。病例和对照中吸烟、洗手、看电视、用电脑的情况差异无统计学意义,但吸烟者患病的相对危险度为不吸烟的4.25倍。结论本起疫情为1起输入性病例引起的甲型H1N1流感暴发,因警员围坐吸烟行为引起病毒的传播。防止飞沫传播在流感大流行中具有重要意义。

One Pandemic Influenza A/H1N1 Outbreak in A Police Force

Objective To investigate the magnitude,source of infection and mode of transmission of an outbreak of pandemic influenza A/H1N1 occurred in one police force of Sichuan Province.Methods We identified the cases,and conducted active case finding and case-control study.Results The primary case,who was close contact in another confirmed outbreak of pandemic influenza A/H1N1,developed disease when he came back.Other 57 polices got ill after that,the attack rate was 20%.Cases were mainly in the freshman polices.The attack rates had no significant difference among polices in different squadrons,squads and floors,but we detected dormitory aggregation.The smokers had more risk to get influenza A/H1N1 than non-smokers(OR=4.25),but behavior of washing hands,watching television and using computer together was not the risk factors.Conclusion This outbreak was caused by an imported case,and transmitted mainly via droplet transmission when polices were smoking and talking with others.Preventing droplet transmission has great significance during a pandemic.
